     If you are the one ordering the book and the book is to be sent to you, fill out only the top address. If you are the one ordering the book and would like it sent to a different address as a gift, include your name on the top and the "Ship to" address on the bottom. They will receive a postcard stating that the gift was from you.

Sales tax
     Total for books x .055 = sales tax.

Shipping and handling
     Book orders will be shipped "Book Rate". Add $3.50 shipping and handling per copy.
